Advocacy and Complaint Support

Dot Lake Village believes every Tribal member and community member deserves respectful, timely, culturally responsive, and professional services. We offer a safe, accessible way to raise concerns, ask questions, or submit a formal complaint regarding services, treatment, communication, or administrative processes.

Our goal is to advocate for accountability, fairness, and improved services for our people and communities.

What Can Be Reported?

Concerns or complaints may include issues related to:

Access to Services

  • Delays in obtaining services or referrals
  • Excessive wait times
  • Scheduling difficulties
  • Inability to obtain medical information
  • Lack of available specialty services
  • Limited geographic access to care

Communication & Professional Conduct

  • Disrespectful or insensitive behavior
  • Failure to listen or provide adequate time
  • Inadequate explanations or education
  • Delays in communicating results or information
  • Culturally inappropriate communication

Coordination of Care

  • Failure to follow up
  • Lack of communication between providers
  • Missing or unavailable information during care
  • Referral delays
  • Poor coordination of treatment or services

Technical Competence & Appropriateness

  • Delayed or incorrect diagnosis
  • Incorrect testing or procedures
  • Inappropriate treatment
  • Procedural errors
  • Failure to refer services outside a provider’s expertise

Facility & Environmental Concerns

  • Unsafe or unclean conditions
  • Equipment problems
  • Inadequate accommodations
  • Infection control concerns
  • Administrative or clinic-level operational issues

Administrative Concerns

  • Tribal member processing issues
  • Billing or claims concerns
  • Failure to comply with applicable Federal or State regulations

Confidentiality & Privacy

Individuals submitting complaints may choose whether their name can be disclosed during an investigation. Dot Lake Village will make reasonable efforts to protect confidentiality while reviewing concerns and responding appropriately.

How to Submit a Complaint

You may submit a complaint by completing the Complaint Form online or downloading the PDF version below and and emailing it to:
council@dotlakevillage.org

Please include as much detail as possible, including:

  • Who was involved
  • What happened
  • When and where the incident occurred
  • Witnesses or supporting information
  • How the issue impacted you or your family

Supporting documents or additional pages may be attached to your submission.
